UC Davis Health System, Patient Services-Transfer Center, located in Sacramento, is seeking a Transfer Case Manager to be responsible for coordinating acute transfers and direct admission requests from physicians, surrounding acute care hospitals, Emergency Departments, Primary Care Network sites, clinics, skilled nursing facilities, and other locations. Incumbent will triage via telephone, in-depth clinical information in order to accurately assess patient condition.  Responsibilities also include collaboration with the MOD, Physicians, Nursing Supervisor, and Nurse Managers to assure appropriate system resource availability, and coordination of internal and external services to ensure optimal continuity of patient care.

Possession of current RN license preferred (BSN highly desirable).  Recent experience in an acute care setting required.  Previous experience coordinating acute transfers and direct admissions preferred. Proven knowledge of State and federal regulations regarding patient transfers and knowledge of Medicare, Medi-Cal, and HMO legislation; MI county contracts, and third party payer contracts with experience in how these programs and regulations effect the functions and role of the department.  Proven knowledge of medical and nursing treatment plans, levels of care, and area resources with skills in assessing the appropriateness of acute medical admissions.  Demonstrated high level of clinical judgment, in-depth assessment skills of patient transfer needs.  Experience with computers and software operations sufficient for retrieving and entering data.
