Kindred Hospital Delaware County, located at 1500 Lansdowne Ave, 5th Floor, Darby, PA 19023 is seeking a Social Worker Case Manager to coordinate management of care for our patients; follow patients throughout the continuum of care and ensure optimum utilization of resources and service delivery. This Social Worker will provide ongoing support and expertise through comprehensive assessment, care planning, and plan implementation for our patients. The Social Worker Case Manager will promote continuity of care and cost effectiveness through case management, utilization review and management and discharge planning. Interested candidate must posses a minimum of three years clinical experience as well as experience in Case Management, Utilization Review and/or Discharge Planning.
