We are actively recruiting for a Regional RN with multi-facility experience in long term care to work as a Signature CARE Consultant (SCC) in our SERENITY (Behavioral based) division.  We are seeking someone to work in partnership with 6-7 SERENITY based long term care facilities currently in TN, FL and we are expanding into KY.

What is behavioral healthcare and how is it different from mental health care?

 

Behavioral healthcare is a multi-disciplinary approach to mental health treatment. Rather than emphasizing either a “medical model” or a “social model” of mental illness, behavioral healthcare recognizes that mental distress has many different causes, and many different disciplines and approaches have a part to play in the treatment process.

 

Summary:  Ensure that clinical systems and standards of care are in place to achieve clinical, regulatory, and reimbursement compliance.  Provide oversight and guidance to staff in assigned facilities which includes role modeling and teaching clinical skills necessary to administer high caliber patient care on a consistent basis. Provide clinical supervision and problem solving assistance to facility staff in functional and physical assessment, care planning, implementation and evaluation of care. Evaluate the effectiveness and appropriateness of outcomes of patient care in collaboration with the facility staff within accepted standards of nursing/clinical practice. 

 

Essential Duties & Responsibilities:

 ·         Integrate the Signature HealthCARE Mission, Vision, and guiding principles within site visit approaches, planning, and making recommendations for change.

·         Analyze Tool Box data for assigned facilities and use the data to prioritize site visits and visit plan for regular as well as for Facility Advisory Board (FAB) approaches.

·         Evaluate efficacy of patient care in terms of physical and functional assessment, care planning and documentation of patient response to care provided.

·         Work closely with other clinical disciplines to develop state-of-the-art methods to improve overall patient care delivery and outcomes.

·         Assist facility staff in systematic problem solving to resolve variances in patient care, regulatory and reimbursement compliance..

·         Communicate and collaborate regularly with facility staff, regional team, State VP of Clinical Operations, and home office via email, facsimile, and telephone.

·         Educate facility staff in effective process improvement techniques.

·         Plan, actively participate in orientation, and provide immediate and continued support for Directors of Nursing. 

·         Assist facility staff with development and implementation of clinical standards of practice in accordance with risk management, clinical and regulatory guidelines, and performance improvement.

·         Guide staff in collecting information for Additional Developmental Requests (ADRs) and appeals, writing realistic plans in response to survey results (2567), and developing Informal Dispute Resolutions (IDR).

·         Demonstrate current, working knowledge of Medicare, Medicaid, RAI/MDS/PPS and Skilled Facility regulatory guidelines at State and Federal level.

·         Update field and home office staff on current CMS and State specific memoranda and regulations pertaining to compliance and regulations.

·         Participate in follow-up and evaluation of the regulatory and RAI/MDS/PPS process.

·         Assist with selection and evaluation of Nursing Administration team in each facility.

·         Utilize home office resources in Clinical Reimbursement and Regulatory Compliance to assist with orientation, program development, and implementation of Quality Improvement initiatives

·         Prepare facility visit report to be reviewed while on-site with administrator and DON.

·         Participate in regional and home office meetings.

·         Assist in other Signature facilities when needed.

  

Requirements

Job Requirements:

·         Registered Nurse in good standing with the required license for each State in which the employee will be consulting.

·         Minimum two (2) years experience in geriatrics/long-term care.

·         Demonstrated intermediate to advanced skills in Microsoft Word, Excel, Power Point and Outlook, Internet and Intranet navigation.

·         Knowledge of state and federal regulatory requirements, reimbursement requirements, and compliance processes associated with each.

·         Highest level of professionalism with the ability to maintain confidentiality.

·         Effective verbal and written English communications skills.

·         Ability to communicate at all levels of organization and work well within a team environment in support of company objectives.

·         Customer service oriented with the ability to work well under pressure.

·         Strong attention to detail and accuracy, excellent organizational skills with ability to prioritize, coordinate and simultaneously maintain multiple projects with high level of quality and productivity.

·         Strong analytical and problem solving skills.

·         Ability to work with minimal supervision, take initiative and make independent decisions.

·         Ability to deal with new tasks without the benefit of written procedures.

·         Approachable, flexible and adaptable to change.

Physical and Sensory Requirements: Moderate physical activity: Requires handling of average-weight objects up to 25 pounds. Standing and/or walking for more than four (4) hours per day. Bending and/or stooping for more than one (1) hour at a time. Function independently, and have flexibility, personal integrity, and the ability to work effectively with stakeholders and vendors.
