Care ManagerPatient Care Coordinator

Care Manager:  To provide direct skilled nursing care and care management for home care patient’s as part of interdisciplinary team under physician’s orders.

Patient Care Coordinator:  Assumes responsibility and accountability for the provision of patient care according to professional standards of nursing in compliance with state and federal laws and regulations. Implements nursing policies and procedures in accordance with the homecare philosophy and all state and federal regulations. Position will conduct screenings, admissions, education and on-call functions as assigned. Utilizes resources in an efficient and economic manner to ensure each patient achieves the highest practicable physical, mental and social well being. Performs any and all professional nursing duties as determined by qualifications and training.

Qualifications:

Education: Registered Nurse with current active state license and a graduate of an accredited school of nursing.

Experience: 1 year professional experience in a hospice or homecare setting.

Skills: Strong interpersonal, leadership, organizational, and clinical skills
