We currently have an opportunity for a professional RN to join our interdisciplinary care team as a Field Case Manager to patients and families throughout New Castle and Kent Counties. Qualifed candidates must be Delaware licensed RNs with two or more years of work experience. Previous hospice, oncology and/or homecare experience a plus but not necessarily required.
