Provides telephonic condition and disease management counseling, healthy lifestyle counseling and educational services to members identified as needing such services. Conducts assessments, creates and implements plans of care and monitors the progress and needs of the member throughout the process. Plays an integral role in the research and development of new programs and educational materials.
REPRESENTATIVE JOB DUTIES : Assist in the screening and identification of members. Perform telephonic health assessments to identify members at risk who would benefit from telephonic health coaching. Assess current health status, satisfaction of/with life and stage of readiness to change with standardized assessment tools. Develop individualized plan of care with member centric goals. Implement, monitor, and evaluate progress toward goals and reassess the effectiveness of the plan. Collaborate with members of the health care team and family members to ensure the appropriate plan of care is in place to best meet the members needs. Refer the member to case management services and community services as appropriate. Engage members and provide support and education to assist members in improving self-management skills, and quality of life in order to achieve an optimal level of wellness. Promote independence through education and empower members to make informed health care decisions. Promote member and provider satisfaction. Provide continuity and consistency of care by building positive relationships through collaboration with members, their families, and providers. Identify quantifiable, measurable outcomes, which include quality of life improvement, improved knowledge, increased adherence, and healthy behavior changes. Advocate for members and their families to facilitate positive outcomes. Responsible for special projects as needed that will contribute to the quality improvement process and overall department efficiencies. Assume an active role in researching, developing and evaluating programs for members with chronic diseases and conditions. Identify and report recommended programmatic changes for enhancing programs. Provide clinical expertise and knowledge.
KNOWLEDGE, SKILLS & ABILITIES REQUIRED: Knowledge of: Individual leadership skills. Adaptability and flexibility in a constantly changing environment. A high degree of professionalism. Technical Skills: Basic PC skills. Highly developed interpersonal skills in order to interact effectively with internal and external customers. Persuasive skills required to negotiate, develop, influence, motivate, and move others through the stages of behavior change. Critical thinking and problem solving skills. Ability to: Handle multiple priorities. Work independently as well as in a team environment. Exercise sound judgment. Take initiative in finding solutions to difficult and/or sensitive problems. Perform other duties as assigned.
QUALIFICATIONS: Minimum Education and Experience: Must have a current, unrestricted Rhode Island Nursing License. Five or more years experience in acute inpatient, rehabilitation, sub acute, skilled facility, home care, or managed health plan experience.
Preferred Education, Additional Qualifications and Experience: Bachelors Degree preferred. Minimum of one year experience in working in a managed care/health maintenance organization.
