Position Description:
The Health Coach is responsible for primary contact and liaison activities with both the qualified Medicaid recipients and community based providers in the surrounding areas of Rutland, Vermont. This program engages in a collaborative approach with providers to ensure members are accessing services in an appropriate and timely manner. Qualified members to complete HRA’s and be stratified into risk categories to determine appropriate level of intervention, education, and coaching to best manage their chronic care conditions. • Providing telephonic chronic care management, training, medical record reviews and other clinically based activities as assigned, based upon recipients severity of condition • educating the Medicaid population about their disease state as well as other health issues relating to their plan of care • Collecting clinical data and entering the information in APS CareConnection® for tracking and metrics reporting purposes • Communicating with providers in the community regarding the consumer’s treatment needs and developing appropriate care plans for specialized needs of members; • Assisting with Provider Education; • Providing face-to-face interventions with high risk beneficiaries; • Interacting with beneficiaries in other risk groups as needed; and • Other duties as assigned. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions EDUCATION REQUIREMENTS: • Registered Nurse with active Vermont license. EXPERIENCE: • Minimum of 3 years experience in related public sector medical services including experience with quality improvement, medical record reviews, chronic/disease care management and/or primary care case management; • Experience managing members with chronic illness, behavior modification, goal setting, and providing constructive feedback preferred; • Strong verbal and written communication skills, prior experience in a training or educational capacity strongly desired; • Previous experience in delivering care management services to the Medicaid population preferred; • Above average computer/technical skills; • Exceptional communication skills with a customer service focus; and • Ability to resolve problems in a non-threatening manner.
