Due to a promotion, Brookdale Senior Living is looking for a nursing professionl for four communities in the St. Petersburg area.  Freedom Inn Bay Pines, Freedom Inn at Tarpon Springs, Freedom Inn at Countryside and Bahia Oaks.

RN Case Manager - St. Petersburg, Clearwater, FL

As an RN Case Manager, we’ll rely on you to provide the coordination of healthcare delivery for each resident to determine appropriate care through management of clinical services and assessment processes, participate in personal service plan reviews with the leadership team, residents and families, regulate medication and health-related supplies, distribution, processes, and ensure proper and timely documentation of healthcare according to company, federal and state requirements.

As a leader within Brookdale,  you will assist the Executive Director's in communicating with family members regarding changing health care needs of residents, deliver health-related CORE, in-service and medication management training, delegate nursing tasks to direct care staff in compliance with the Nurse Practice Act in areas of basic health care and proper documentation.

Requires:  RN state nursing license and a minimum of 2 years experience working with elderly, working knowledge of personal computing and word process software preferably in a Microsoft Windows environment is required.
