Full time RN Case Manager position in the Macedonia area.   Manage a case load  of patients working with an interdisciplinary team.  Day time positions.  Manage and monitor pain and symptom issues.  Conduct admissions, inservices, and provide education.  Good understanding of geriatrics and hospice is beneficial.  Hospice experience preferred.
