Requirements:
New Jersey - RN Licensure, Previous Case Management, UR  or related experience required.   

Duties:
Assess patient care and treatment needs, develop, monitor, and evaluate treatment plans and patient progress, facilitate interdisciplinary approaches, admits new patients by reviewing medical record, determines patient clinical  requirements by completing through intake interviews, determining the need for therapeutic medical, psycho-social, and psychiatric evaluations, review therapist evaluations, treatment objectives, and plans.
