Established and Respected Medical Center located in the Upper Eastside of Manhattan is looking for RN Care Coordinators with strong clinical and case management experience!!

 

RN Coordinator/Nurse Case Manager

Under the supervision of the site manager, the Care Coordinator is responsible for coordinating and integrating high quality, cost effective care delivered to patients across an episode of illness or throughout the entire continuum of health care delivery.

Assesses the patient's clinical status on admission utilizing Milliman and Robertson or InterQual Guidelines for admission and continued stay to identify needs, issues, resources, and care goals to achieve desired outcomes Screens and assesses patients through the use of the New York State Department of Health Patient Review Instrument (PRI) Works with Social Work on the completion of the PRI as needed Coordinates activities of the interdisciplinary team members focusing on effective management of care across the continuum from admission to discharge based on established LOS guidelines Ensures that discharge orders, prescriptions, DME and follow up appointments are made or completed by the appropriate parties in a timely manner prior to discharge Provides clinical information pertinent to the patient's stay in the hospital as outlined in the provider agreements negotiated with the Managed Care Department maximizing reimbursement to the hospital for payers with whom the hospital contracts as a provider

Minimum Qualifications

RN with BSN or other equivalent degree, Master degree preferred Five year's clinical experience in an acute care setting Two years case management in an inpatient, HMO, Insurance or other outpatient setting preferred  Previous experience working with lower gastrointestinal procedures

 

Salary pays up to $82,000 with excellent benefits!!
