Performing the initial admission assessment and establishing the initial plan of care
Communicating with physicians and other team members to confirm, correlate and update patient plan of treatment and provide physician with a status report on a regular basis
Providing on-going assessment of patient response to treatments, medications and teaching and applies interventions as appropriate.

Pool provides support after hours and on weekends. This position provides coverage for our Lake Geneva office and serves southern Wisconsin. Mileage is reimbursed but willingness to travel is a must!General QualificationsAssociate’s Degree in nursing.
Minimum two years of home health/hospice experience or demonstrated competency in direct home health or hospice care
Current licensure to practice as a Registered Nurse in the state of Wisconsin.
