In the role of Home Care RN with CEI, you will be an intregal member of a specific interdisciplinary team, and will be based out of one of our Oakland sites. Your primarily responsibility will be to regularly travel to the homes of our participants for conducting periodic in-person assessments and supervisory visits, with the core function of providing high-quality skilled nursing care to our program participants. Your prior background and knowledge of clinical decision-making for a frail and elderly population will be directly utilized for implementing primary care orders, evaluating participant complaints, developing appropriate plans of care, and for providing participant, caregiver and family care instruction. You will regularly rely on your understanding of home health agency protocol for deciding proper care assessment and for identifying safety hazards and taking the necessary precautions to establish a safe home environment for participants. Having the ability to work independently will be crucial, in addition to your meticulous organization and prioritization skills, as you will structure your own schedule for managing multiple patient cases, including overseeing their home health care needs.
