Position Title: Hospice Registered Nurse Case Manager
Position Summary:
Manages a caseload giving consideration to patient/family needs, travel efficiency and service priorities. Plans, implements and evaluates individual occupational therapy programs and diagnostic tests in conjunction with the prescription and/or the direction of a physician. Treatment planning in consultation with the physician and IDT to achieve goals for hospice clients and their families Instructs patients and family members in supportive, preventive and therapeutic procedures. Provides consultation to other Agency staff concerning the application of professional principles and procedures to problems in patient care. Consultation with IDT members.Identifies barriers to desired outcomes. Evaluates equipment needs to increase functional level.Accountable to the Director of Rehabilitation for attainment of standards of professional practice.Completes records and reports necessary for patient service and administrative requirements as determined by the Agency.Assumes responsibility for professional growth and maintains clinical competence.Establishes and maintains effective working relationships with patient, families, peers, supervisors and community agencies.
Reuirements:
Possession of the appropriate licensure, certification and/or registration.Minimum of one year of direct care patient experience, preferably of a generalized nature.Current driver’s license.
Willing and able to travel to patients' homes throughout the entire city of Philadelphia. There are times when patients' homes are in the surrounding towns of Montgomery and Delaware Counties, as well.
Must have completed an educational program (Bachelors or Masters Degree) currently approved by the respective national professional organization
