Providing a full spectrum of home based health services enables us to coordinate care across Under the supervision of the Area Director, the Clinical Manager is responsible for the overall management of a home care location for Sutter VNA & Hospice, including one to three supervisory teams, with up to a total of up to approximately 600 patients.  This position assists the Area Director with the planning, organization and direction of patient care services in accordance with current standards and regulations and subsequently, the clinical staff, ensuring that optimal degree of quality care is maintained.  High level management skills needed to be successful in the role as leader include conflict management, negotiation, team building, communication, budget administration and coaching/mentoring others.  Responsibilities include overall responsibility for coordination of patient care services furnished including the development of personnel qualifications and the assignment of personnel; the direct supervision of Clinical Supervisors; assistance with hiring, orienting and evaluating qualified personnel for all disciplines; general oversight of patient care and effective management of utilization of services; coordination of agency functions; fiscal management, and representation of the local site (and the agency as a whole) to the Sutter Health system and to the local community at large. The Director of Patient Care  Services (or their designee) shall be available on the premises or immediately accessible by telecommunications during operating hours when the patients are receiving services.  Other managerial responsibilities as assigned.
Education:
A registered nurse with a baccalaureate or higher degree in nursing or other health related field with three years of experience within the last five years in a home health agency, primary care clinic or health facility, at least one year of which was in a supervisory or administrative capacity, strongly preferred;
OR
A registered nurse with four years experience within the last five years in a home health agency, primary care clinic, or health facility, at lease one year of which was in a supervisory or administrative capacity, strongly preferred
Licensure:
Current California RN license or professional licensure in another health-related field; valid California drivers license; current auto insurance; current CPR certification.
Experience:
A minimum of three years experience within the last five years in a home health agency, primary care clinic or health facility, at least one year of which was in a supervisory or administrative capacity strongly preferred. Progressive clinical and management experience in the home health and/or home Hospice settings as typically expected, with a minimum of three years of in progressively responsible home health management positions strongly preferred. Sufficient background knowledge and expertise in clinical decision-making for the patient population of the home health agency to meet the needs of the patients and to contribute to Quality Management review and evaluation.
Knowledge:
Excellent understanding of home care services, Title XXII, Medicare Conditions of Participation, JCAHO home health accreditation requirements, OASIS, QI outcomes/benchmarking, infection control; good understanding of reimbursement patterns for Medicare and other payers; good working understanding of fiscal responsibilities, budget and financial reports.
Special Skills/Equipment:
Self-starter with a high degree of initiative, motivation, flexibility, energy and creativity.  Ability to form harmonious working relationships with internal and external customers, and able to facilitate positive and productive working relationships among     co-workers and subordinates.  Exhibits excellent oral and written communication skills; ability to operate basic computer work processing, understanding of the principles of adult education and training; collaborative and problem solving skills.  Access to a reliable automobile.
