Community Support Specialist Paraprofessional

Primary Purpose of Position:

The primary purpose of this position is to provide one-on-one interventions within the community to develop interpersonal and community coping skills, including adaptation to home, school, and work environments; therapeutic mentoring; symptom monitoring; monitoring medications; and self management of symptoms to meet the mental health and substance abuse needs of consumers who because of documented functional and/or behavioral deficits are at risk of intensified disability and/or have an inability to live successfully in the community without specific support and guidance, and provide input into the Person-Centered Plan modifications..  Interventions are provided to facilitate improvement in all areas of functioning as identified in service plans and assist the consumer in acquiring and maintaining the skills necessary to independent sustained improved functioning and remain in the community.

Duties and Responsibilities:

1.        Various skill building activities to include:

(a)      Training of caregiver

(b)      Daily and Community Living Skills

(c)      Socialization Skills

(d)      Adaptation Skills

(e)      Sympton Monitoring and Management Skills

(f)       Development of leisure time and activities

(g)      Education substance abuse

(h)      Therapeutic mentoring

(i)        Behavior and anger management techniques

*Responsible for documenting daily progress notes of specific intervention services provided to clients to insure that documentation in service records meets the requirements as specified in Medicaid Service Guidelines.

Minimum Requirements for ParaProfessional Position:

High school diploma/GED

Minimum Requirements for Associate Professional Position:

Four-year college degree

Upon hiring applicants must submit the following paperwork:

·         Office of the Inspector General (OIG) Exclusion Database Check

·         Proof of education/training

·         North Carolina Health Care Personnel Registry Check

Also, the following training classes must be updated annually upon hire.
