Imagine yourself working with a multi disciplinary team consisting of other Physical Therapists, Occupational Therapists, Speech Therapists, Physical and Occupational Therapist Assistants, and other professional medical and office staff. We are looking for a physical therapist to work in our dynamic hospital setting. If you have strong communication skills, are service oriented and an excellent team player, you will enjoy working with our multidisciplinary team! This position would see a variety of diagnosis including: CVA, medically complex, neuro, multiple trauma, ortho, and medical/surgical. It offers a supportive leadership team and collaborative environment where employees are encouraged to share their unique talents and ideas. Imagine yourself –
· Interpreting physician referrals and completing musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations utilizing the appropriate methods.
· Based on the evaluation results, planning and implementing individual treatment based on the functional goals and appropriately adjusting the individualized treatment plan based on the patient’s response to their treatment.
· Demonstrating your skills in the use of modalities and exercise equipment.
· Participating in performance improvement activities designed to identify and resolve problems affecting patient care outcome and to improve the quality of care.
· Planning for and completing patient discharge including providing written home programs, patient/family teaching, referral for community services and ordering equipment.

· Wisconsin State Licensure required.
· Must obtain CPR certification within three months of hire.
· Demonstrated knowledge of indications and contraindications of modalities and exercise. Demonstrated skill in use of modalities, exercise equipment and other devices.
· Excellent time management and multi-tasking skills, organization, problem solving, and decision-making skills required.
· The ability to work self-directed and as a team. Customer service skills are essential.Desired Qualifications· Excellent communication skills
