Walnut Street in Milwaukee. This position would give you exposure to both the home care and day care setting. Providing treatments to patients based on the Physical Therapist’s evaluation. Treatments include exercise, gait training, mobility and transfer training, balance and coordination activities, use of modalities, soft-tissue/joint mobilization, and orthotic/prosthetic training.
Managing your own caseload, including scheduling and informing your supervisor of coverage needs.
Collaborating with members of the interdisciplinary team which includes; other Therapists, Nurses, Home Health Aides, Social Workers, Medical Directors, Physicians, and a wide variety of other professionals.
Providing on-going assessment of your patient’s response to treatments and teaching.  
Teaching appropriate treatment procedures and supportive activities to patients and caregivers, and developing a plan for obtaining adaptive devices that your patient needs.

General Qualifications
Completion of two-year Physical Therapist Assistant Degree from an APTA accredited school.
Wisconsin State Licensure required. 
 Must obtain CPR certification within three months of hire. 
Excellent human relations, organization, problem solving, teamwork, and decision-making skills required. 
 Customer service skills are essential.
Desired QualificationsHome Care experience desired, or one-year of experience as a Physical Therapist Assistant.
