Position Purpose, Nature and Scope:
This position will augment the Cardiac Transplant and Heart Failure Program by providing clinical support and expertise to heart transplant and heart failure patients and their families. The practitioner must be able to participate in the management of heart failure, heart transplant, and mechanically supported patients from the Intermountain West and beyond. At times, the incumbent will serve as the first team member to meet with patients and their parents, and must adequately reflect the approach to care and philosophy of the team. This professional works in collaboration with the heart failure/transplant cardiologist(s), other members of the transplant team, other cardiologists, intensivists, CV surgeons, residents, fellows, researchers, other advanced practice clinicians and nurses. The Cardiac Transplant and Heart Failure Program at Primary Children's is comprised of an exceptional team of professionals working in the premier pediatric heart center in the 5 state intermountain region. Essential Job Functions and Performance Standards:Inpatient responsibilities:Reviews patient records from within and beyond our system Performs H&P examinations daily, plus PRN assessments. Participates in the inpatient care of pre-transplant, post-transplant and Heart Failure patients Will assist the attending cardiologist in managing the care of mechanical circulatory supported patients (VADs, Balloon pump, ECMO) Orders and interprets appropriate laboratory and other studies as indicated Contributes to the patient plan of care: notes and reports patient progress, response and complications. Makes appropriate course adjustments and interventions, Documents using the dictation service or Electronic Medical Record as appropriate Divides work load with the transplant physician and other care providers; pre-rounds on assigned patients, then rounds with the team at designated times. Works with nurse coordinator to complete plan. Communicates with patients, families and other involved healthcare providers Outpatient responsibilities:Participates as needed in the initial evaluation of new heart failure patients to the clinic. Help facilitate the care of patients transitioning from the general cardiologist to the heart failure program. Provides care to-followup patients; seen either collaboratively or individually with the attending cardiologist Will assist in providing timely care to patients whose condition requires a same day work in visit, or next day visit for after hour “on call” requests. Documents using the existing EMR Assists transplant coordinator and nursing staff with clinic and telephone triage and laboratory review Call responsibilities:First provider to be called by other team members, patients, and other health care professionals. Includes patient triage, assessment, medical care, coordinating and communicating with attending physicians as needed, complying with all appropriate safety and infection control standards and mandatory compliance activities. Other:Assist the Heart Transplant Coordinator in daily responsibilities and participate in regulatory and quality assurance efforts Attends weekly team meetings to review program patients Provide teaching and education for families and nursing staff Participate in the revision of guidelines and protocols for the heart transplant program. Participate in the revision of guidelines and protocols for the heart failure program. Assist cardiologist as required in the development of the VAD program Attends continuing medical education seminars and conferences to maintain license.Requirements
Current licensure (including controlled substances) in the state of Utah as Nurse Practitioner or Physician Assistant Current Basic Life Support and Pediatric Advanced Life Support (PALS). Experience as a licensed advanced practice clinician preferred. With preference given to individuals with pediatrics, cardiology, cardiovascular or intensive care experience. Excellent written and oral communication skills; ability to work in an extended team environmentAbility to establish and maintain effective working relationships with physicians and hospital staff, including fellows in training.Physical Requirements
