Job Skills/Requirements
As an RN Case Manager you will use our computer and telephone systems to provide patient assessment and education for case management and disease management diagnosis. You will work closely with our patient/members to provide telephone based care in a way that effects behavior changes and positively impacts the patients life.

Primary responsibility includes assessment and education for case management/disease management diagnosis. Your focus will be primarily Neuro case management but in a disease management capacity you will also work with diabetes, cardiovascular, respiratory, and any other potential disease management or case management diagnosis. Responsible for the delivery, over the telephone, of the utilization management process to include: making clinical recommendations regarding medical necessity for admission, continued stay and the development/implementation of a comprehensive case management treatment plan.

Current unrestricted Registered Nurse license in Arizona required. Bachelor's degree in nursing preferred.
Prior case management experience is required, CCM prefered
Bilingual (English-Spanish) preferred.
Appropriate and relevant clinical experience in the past 5 years.


Certification Requirements
 – Active AZ RN License

Education Requirements
 – Associate's degree minimum required
