MDS AND CARE PLAN COORDINATOR
Full-time position available. (EOE/M/F/V/D)


Requirements
RN must be currently registered/licensed with state of South Carolina. Two years of nursing experience required, preferably geriatric nursing. Must have achieved or be willing to achieve the CCC certification. Must have experience with patient care assessments and the care plan process.

Responsibilities
Must be able to oversee patient care assessments and the care plan process for each resident in accordance with all laws, regulations and company standards.
