MDS and CARE PLAN COORDINATOR
Full-time position available. (EOE/M/F/V/D)


Requirements
RN must be currently registered/licensed with state of Tennessee. Two years of long-term care experience preferred. Must have a passion for geriatrics.

Responsibilities
Must be able to oversee patient care assessments and the care plan process for each resident in accordance with all laws, regulations and company standards.
