Responsibilities include:
Provide nursing care to patients in the community. The care is delivered within PHCs Philosophy, Policy, and Standards of Community Health Practice. Qualifications: *Home care experience preferred

*Working knowledge of hand held patient care devices desired

*Demonstrated knowledge and skills necessary to provide care to the geriatric, Pediatric, and/or adult population.

*Ability to communicate effectively and respectfully both verbally and in writing, with People who have varied backgrounds.

*Graduate of an approved program of licensed practical nursing

*Holds a current LPN professional license in Massachusetts

*Minimum of two years previous medical-surgical nursing experience
