SALARY $50-60k

80% INTERNAL, 20% HANDS ON PATIENT CONTACT

Is seeking a dynamic, self motivated individual to join our growing organization as Social Worker in our Brooklyn office in Canarsie. The Social Worker promotes and maintains the mental and social health of frail, elderly enrolled members through assessment, treatment, teaching and counseling. The Social Worker provides basic casework and consultation for members and facilitates communication between members, their family, social and medical services, and the Care Team. The Social Worker is responsible for the implementation of social work care plan and coordination of social work with other services. Requirements: · Requires MSW.

CANTONESE/MANDARIN SPEAKING. Computer skills required. · Ability to work within an interdisciplinary care management team to provide services to frail elderly. · Must be flexible and able to relate well with seniors and their families to deal with sensitive issues and facilitate problem solving.
