Position Description:
POSITION: HRA (Health Risk Assessment) Coordinator DEPARTMENT: Outreach LOCATION: Williston, VT DESCRIPTION: The HRA Coordinator serves as a liaison for the eligible Vermont Medicaid recipients with respect to our Disease Management Services. This is a telephonic position, and is responsible for assisting and educating Medicaid members on the Program parameters, objectives, and benefits. This Program engages in a collaborative approach with providers to ensure members are accessing services in an appropriate and timely manner. The main function is to assist Medicaid recipients in completion of Health Risk Assessments (HRA’s) in order to be appropriately stratified into risk categories to determine appropriate level of intervention, education, and coaching to best manage their chronic care conditions. • Assist Medicaid members in the mechanics of how to complete HRA’s. • Respond to inquiries from members regarding the HRA process. EDUCATION: Associates Degree, Bachelor’s Degree in a healthcare or related discipline preferred. REQUIREMENTS: . 2 year support or customer service experience in a healthcare setting; • Background and experience in health care/disease management as well as community systems and resources, and/or data collection preferred; • Previous experience in delivering care management services to the Medicaid population preferred; • Above average computer/technical skills; • Exceptional communication skills with a customer service focus; and • Ability to resolve problems in a non-threatening manner. • Ability and experience working with a diverse population.
