This is an exciting opportunity to use your clinical experience to help people during the last stages of their lives. As an Evercare Hospice Case Manager, RN, you would visit patients in their homes or nursing facilities and make recommendations for their care. If you’re ready to take your nursing career in a new direction while enjoying the benefits a Fortune 100 company offers, we encourage you to apply.   Primary responsibilities include: Visiting hospice patients in their homes or nursing facilities in the Denver area Making assessments and recommending patient care and support Providing assistance to patients’ families Coordinating efforts with care givers and staff Overseeing patients’ hospice benefits Documenting all assessments and communication Participating in team meetings Developing care plans and strategies A current, unrestricted RN license in the state of Colorado is required. RNs with clinical or hospice experience will be preferred. A background in discharge planning is a plus. Excellent communication skills are critical.
