PHYSICAL THERAPIST 

 

Responsibilities:

He/she is responsible for:providing skilled physical therapy care to patients with impairments, functional limitations, disabilities, or changes in physical function resulting from injury, disease, or other causes; coordinating other patient care activities/services to provide the highest quality of patient care in the geographic service area to which he/she is assigned. He/she works under the supervision of the designated manager.

Qualifications:

1. Graduation from a physical therapy school accredited by the Commission on Accreditation in Physical Therapy Education.

2. Licensed or registered as a Therapist in the state of Maryland

3. A minimum of one year of experience in acute, rehab, or long term care setting required; previous home care experience preferred.

4. Must possess strong patient assessment and patient management skills.

5. Absolute integrity and the use of good judgment to handle the professional and confidential nature of assignments.

6. Speaks, writes, reads, and comprehends English.

7. Must be willing to use Web-based hand held point-of-care device.

 

 

HOME CARE REGISTERED NURSE

 

Responsibilities:

 

He/she is responsible for providing direct patient care to ensure the effectiveness of the care, treatments and services being provided by the agency, meet the patient’s needs. Admit patients to the services, completing appropriate clinical documentation under the Clinical Manager’s direction. Assess individual status and care/instruction needed by the patient. Administer on-going care and case management for each patient, providing necessary follow-up as directed by the Clinical Manager. Confer with physician and develop the initial plan of treatment based on physician’s orders and initial patient assessment. Provide hands-on care, management and evaluation of the care plan, and education of the patient in accordance with physician orders, under Clinical Manager’s supervision. Revise plan in consultation with physician based on ongoing assessments and as required by policy/regulation.

Qualifications :

 

1.A Registered Nurse graduated from an approved school of nursing with a minimum of one year of acute care nursing experience required; previous home care or hospice nursing experience preferred.

2. Licensed as a Registered Nurse in the state of Maryland.

3. Must possess strong clinical assessment, evaluation and patient education skills.

4. Must possess current knowledge of clinical techniques and procedures and be able to effectively perform the duties and tasks outlined in the patient’s care plan.

5. Previous experience in venipuncture skills and intravenous therapy administration preferred, and/or specialty experience in area offered by agency/care team.

6. Excellent organizational, interpersonal, telephone, communication, and documentation skills.

7. Ability to adapt to changing organizational needs.

8. Speaks, writes, reads, and comprehends English.


9. Must be willing to use Web based hand held point-of-care device.

 

OCCUPATIONAL THERAPIST


Responsibilities: He/she is responsible for: (1) providing skilled occupational therapy to patients whose meaningful and purposeful occupations (activities of daily living, work and productive activities, play and leisure) have been disrupted by injury, disease, or other cause; (2) recording clinical and progress notes; and (3) coordinating other patient care activities/services to provide the highest quality of patient care in the geographic service area to which he/she is assigned. He/she works under the supervision of the designated supervisor.


 

Qualifications:

1. Graduation from an occupational therapy program accredited by the Accreditation Council for Occupational Therapy Education.
2. Licensed in the state(s) in which he/she is required to practice.

3. A minimum of one year of acute, rehab, or long-term care experience required; previous home care experience preferred.

4. Excellent organizational, interpersonal, telephone, communication, and documentation skills. 

5. Ability to adapt to changing organizational needs.

6. Speaks, writes, reads, and comprehends English.

7. Must be willing to use Web-based hand held point-of-care device.
