HCR Manor Care provides a range of services, including skilled nursing care, assisted living, post-acute medical and rehabilitation care, hospice care, home health care and rehabilitation therapy. Individual will lead dynamic Social Services team within the facility. Responsible to provide medically related social work services so that each resident may attain or maintain the highest practicable level of physical, mental, and psychosocial well-being. Locations: 484 - MCHS (Overland Park) SNF/ALF, Overland Park, KS , Overland Park, KS 2 - 3 years of social work supervised experience working directly in a health care setting.
