Position Profile

The Director of Case Management is responsible for developing and implementing a new, more highly integrated model of case management throughout the facility for both inpatients, as well as outpatients.  The position reports to the Vice President of Patient Care Services and oversees approximately 12 FTEs, including an RN case manager, a case manager/clinical documentation specialist and a social worker within the case management department, as well as providing oversight for the float pool and the staffing office.  Qualified candidates will be bachelor’s prepared nurses with two to three years of leadership experience in an acute-care hospital setting, and at least five years experience in case management.  The ideal candidate will have exceptional interpersonal and communication skills, a collaborative leadership style, and proven success in cultural change management.  Experience in implementing an integrated case management model is a plus.  Bachelor’s degree and current RN licensure are required; Master’s degree is preferred.

This is a 49-bed, faith-based, not-for-profit, Joint Commission accredited hospital offering services in general medical and surgical care, intensive care, pediatrics, obstetrics, orthopedics, emergency medicine, and trauma care.  The hospital is a member of Catholic Health Initiatives, one of the largest non-profit healthcare systems, with more than 60 acute care hospitals nationwide.  Through a community partnership the hospital provides care in an outpatient medical clinic to residents of the surrounding communities.  Services offered include an immediate care facility, orthopedic rehabilitation, a drive-through pharmacy, primary care physician offices, routine check-ups and immunizations, sports medicine, diagnostic imaging, clinical breast exams, and occupational medicine.
