Are you looking for a great career growth opportunity? Join the Care Coordination Team as a Director of Care Management and Throughput at Swedish Medical Center! In Care Coordination, the patient is at the center of everything we do. Our goal is to give the right care at the right time to every patient we serve. In this important role, you will be responsible for assuring that the appropriate systems and structure are in place to facilitate entry of acute care patients to the appropriate facility and manage appropriate admissions and lengths of stay. We are seeking a leader to oversee relationships with services in the community who care for patients outside the short-term acute care setting. This position is also responsible for studying national and regional trends and adjusting the admit, transfer, and discharge model to meet financial, regulatory, and clinical goals. This position requires a Master’s degree in a health related field; Clinical Nurse Leader Master’s degree is preferred. Must have five years of management experience with at least three years of proven success with change management at the system level. Qualified individuals must possess knowledge of clinical regulations and health finance in relation to patient care management. A current Washington State Nursing license and valid driver’s license are required.
