Doral is adding a Dental Health Care Specialist (DHCS) role to the Client Services team. The Dental Health Care Specialist is responsible for care coordination and case management for members who meet defined criteria. Additionally, the DHCS is accountable for outreach and education to promote oral health and increase member access to quality dental care. Specific job responsiblities include:

Review case management requests and plans from dental providers; evaluate need against defined criteria and determine completeness of plans.Assess referrals for case management from dental providers and health care professionals; develop, implement and monitor case management plans.Coordinate and communicate all case management activities with members/guardians, dental providers, medical providers, MCOs and other health care/service providers as appropriate.Document and report case management plans and their effectivity.Collaborate with Connecticut Project Manager, field-based EPSDT and Outreach Coordinators, and Corporate Wellness and Implementations Administrator in the development of generic and customized outreach strategies for increasing dental access in Connecticut.Develop and implement member outreach and oral health education programs in Connecticut.Collect, compile and analyzes information gleaned from outreach initiatives in order to determine impact, and recommend changes that may make future outreach projects more effective.Maintain and increase dental access rates as required by the Connecticut Department of Social Services (DSS) and measured by the CMS-416 report.Implement prevention and intervention strategies to support increased EPSDT screening and treatment services.Establish and maintain community relationships that provide for participation in health care initiatives and programs to create dental awareness.Develop educational materials and work plans for non-dental healthcare providers, managed care organizations and community organizations.Manage workflows and program budget to meet DSS and Doral goals and deadlines.Provide Doral process training to DSS staff, service organizations, and advocacy groups as appropriate.Submit regular reporting of access performance measures to Doral management.Represent Doral in a professional manner at all times.Complete special projects as requested.

Requirements for this role include knowledge of healthcare advocay activities, knowledge of government programs, knowledge of general computer software. (Excel, MS Word, Power Pint and Access a plus)excellent verbal and written communitcation skills, and the ability to create and deliver presentations to various sized and structured groups. Other requirements include the ability to detail, accuracy and organizational skills, ability to prioritize and adapt to changing priorities. A Bachelor's degree in related field or quivalent experience in social services, education, healthcare or case management is preferred.
