Case Manager, Therapist, Counselor, Rehabilitation, Residential, Recovery

 

Horizon House, Inc. is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues, and substance abuse.   With over 100 locations throughout Southeastern Pennsylvania and the state of Delaware, Horizon House provides services to a diverse population focusing on individual strength and choice.

 

Delaware County locations are seeking experienced professionals for the following positions:

 

Clinical Services Specialist-Will monitor and coordinate client services and procedures, client assessment, rehabilitation planning and implementation of services while ensuring compliance with all funding source regulations and Agency policies and procedures. Participate in the evaluation of residential referrals. M.A. degree in Human Services/Social Sciences, Healthcare or related field preferred.

 

Psychiatric Nurse (PT 8hrs/ Wk Flex Hours)- Provide psychiatric nursing services consistent with physician orders and standard nursing practices. Provide education to clients, non-medical team members and the community. Prior experience in servicing community mental health facilities and psychiatric needs patients. Comprehensive knowledge of psychiatric, clinical and psychosocial rehabilitation practices required.  Experience working with dual-diagnosed and homeless population a plus.

Board Certified Licensed RN in the State of PA required.

 

Certified Peer Specialist-Will provide peer support services, therapeutic interactions designed to promote empowerment, self-determination, understanding, coping skills, and resilience through mentoring and service coordination supports. Assist program participants with mental illnesses and co-occurring disorders to regain control over their own lives and their own recovery process.  Must be a certified Peer Specialist certified by a PA State approved entity.

 

Psych Rehab Specialist- Through the use of a clubhouse model, provide skill/resource development, counseling and encouragement to clients as they move into vocational or community based activities of their choice. Ensure consumers receive the level of and/or have access to the services/supports they need. Monitor and track assigned caseload and participate in the development of individualized rehabilitation plans. 

 

Employment Development Specialist- Participate in the development of vocational opportunities for clients; assist clients in obtaining and maintaining supports from the service delivery system.

 

Residential Support Specialist-Responsibilities include service coordination, counseling and direct support to adults with mental illness/substance abuse problems and/or co-occurring disorders. BA plus two years of case management or other related experience is preferred.

 

The opportunities available require a wide range of educational backgrounds or any combination of experience, education or training that would provide the level of knowledge, skill and ability necessary to be successful working with our consumers.  Valid driver’s license required for all positions.
