STATEMENT OF PURPOSE: Coordinates the admission with other departments by collecting and distributing the appropriate information. Identifies information needed from referring hospital to finalize the admission. Communicates pre-admission information to the Patient Services Department in corporate office. Conducts patient referral services as directed. Major tasks, Duties, and Responsibilities 1. Consistently demonstrates the ability to recognize, establish and deal with admitting priorities promptly. Measured by: Supervisor; external referral sources 2. Effectively assists in planning activities and assesses patient admitting systems and processes, including marketing, clinical program development, and related plans and strategies. Measured by: Supervisor; feedback from co-workers; referral sources 3. Develops and implements marketing plans that promote the services of Kindred Hospital. Measured by: Supervisor 4. Develops in collaboration with Administration targets related to referrals and conversions and attain established targets. Measured by: Supervisor 5. Reviews and assesses the implications of long and short term hospital strategies on the admitting services functions and recommends responsive plans and actions as required. Measured by: Supervisor 6. Effectively participates in the planing of inservice training programs to instruct medical, nursing, and other hospital personnel on admitting related issues and developments. Measured by: Supervisor; feedback from team members 7. Effectively functions as a liaison between the institutional managers in coordinating and participating in interdepartmental program planning and implementation efforts as required. Measured by: Supervisor; follow-up with referral sources 8. Collect, analyze and report statistical data related to Case Manager functions and completes in a timely manner, thoroughly and appropriately detailed. Maintains required reports for department and hospital. Measured by: Supervisor 9. Maintain accurate records of travel and expenses incurred in the performance of the job and submits in a timely manner to Administration. Measured by: Supervisor 10. Self development through affiliation with Professional Associations in order to remain current with trends, changes and legislation in the field. Measured by: Supervisor 11. Promote the hospital’s services and image in an innovative and creative manner in order to market the hospital’s products, programs and services. Measured by: Supervisor; follow-up with referral sources 12. Promotes positive customer service through follow-up and resolution of complaints or problems received from patients, referral sources, staff, other departments, administration of the medical staff. Measured by: Supervisor; follow-up with families, team members and referral sources 13. Demonstrates ability to work effectively with others through development of positive relationships. Maintains confidentiality of patient and employee information. Resolves conflict appropriately and communicates respectfully in accordance with Kindred Standards of Conduct. Measured by: Supervisor; feedback from team members, patients and families. 14. Provides customer service in a manner that utilizes tact, courtesy, respect and good judgement I interactions. Demonstrates ability to assess and accommodate for diverse needs n regard to culture, religion and disability. Measured by: Supervisor; feedback form customers (patients, family, co-workers, physicians) 15. Participates actively in hospital quality improvement process. Measured by: Attendance at departmental meetings and QAS committee meetings as assigned. 16. Participates in the Hazardous Material Program. Hazards include exposure to blood and body fluids, possible communicable diseases, sharp objects and instruments, assorted chemicals and gasses as listed in the Hazardous Materials Program Manual. Promote safety of staff and patients through continuos monitoring and compliance with infection control and safety programs. Measured by: Completion of annual competency. Environmental rounds. Supervisor observation JOB REQUISITES: A). EDUCATION AND TRAINING: Clinical RRT; RN B). LICENSING/REGISTRATION/CERTIFICATION: State and National C). EXPERIENCE: Minimal: 3 years clinical (medicine/critical care in a hospital setting). Prior marketing a plus.
