Responsibilities include:

 

Assist referral sources in identifying patients with care needs that match the services Golden Living Centers provideEducate the potential patient and family regarding the services provided, admissions process, treatment and projected outcomeCommunicate with appropriate nursing facility staff and ensure accommodations and staff are prepared for the patientCoordinate the patient transferComplete necessary transfer and admissions paperworkProvide on-site clinical assessments to referring providersBuild relationships throughout the professional healthcare community

 

Successful candidates wanting to be considered for this position must meet the following requirements:

 

RN or LPN, currently licensed by state in which services are providedMinimum of 5 years of general nursing experienceMinimum of 2 years in long-term careMust have outstanding communication and documented assessment skillsAbility and willingness to work on-site in numerous facilities
