We are currently seeking a Clinical Coordinator for our Children's program in Lawrence, MA.

Find out more about us at: www.thementornetwork.com The Clinical Coordinator will coordinate the development and implementation of all aspects including: support, treatment planning, conducting home visits, liaison between agency and funding sources for individuals receiving services, and offering feedback and oversight for caregivers. To perform this job successfully, an individual must be able to satisfactorily perform each essential function listed below. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. Provide case management and counseling services to a client caseload assigned by the Program Manager Assesses referred clients, leads development of service plan in conjunction with treatment team and funding source; in accordance with corporate, contractual, and regulatory standards Coordinates and evaluates plan implementation Provides monitoring and consultation to mentors regarding work with clients and implementation of service plan Makes home visits and conduct mentor group meetings as required Maintains current and complete client and reporting records in accordance with corporate and regulatory standards Assists with all legal, licensing, regulatory and certification activities as needed Provides pre-service introduction and in-service continuing education to mentors as appropriate per regulatory requirements and treatment plans Secures individual mentor contracts and complete annual contract reviews for mentors Assists with emergency coverage and crisis intervention support for all clients as needed Participates in rotational on-call system In collaboration with the Program Manager and Recruiter, assists in mentor selection, certification, retention and social activities  Participates in all activities designed to enhance and build an active network of mentors  REQUIREMENTS The requirements listed below are representative of the knowledge, skill, and/or abilities required to perform the job. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. Four year college degree in Human Service or Rehabilitation Field One year experience in development disabilities, mental health, child welfare system or adult services Licensure and/or certification in field required as determined by site contractual agreement and state standards Current drivers license, car registration and auto insurance if providing transportation for individuals receiving services Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organization. Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs. Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations. Ability to effectively communicate (read, write, speak and understand) in English Knowledge of the service system Ability to work independently
