SUMMARY DESCRIPTION
Coordinates the development, completion and transmission of the resident assessment in accordance with the regulatory requirements; reviews and manages reimbursement in compliance with SunBridge policies and procedures. Responsible to case manage residents covered by either Medicare and Managed Care insurance programs. Requirements

* Current RN / LPN license in state of employment.
* Demonstrated knowledge of clinical standards of practice, regulations and reimbursement governing long-term care and Medicare participation.
* Ability to function as an effective team member through good communication and cooperation to meet departmental goals, to develop/maintain good personnel relations and employee morale.
* Ability to deal tactfully with residents, family members, visitors, government agencies/personnel and the general public.
* Ability to make independent decisions, to follow instructions, and to accept constructive feedback.
* Ability to understand and monitor applicable regulations, policies and procedures.
* Ability to respect residents including those who have impaired cognition.
* Sufficient verbal and written communication skills to perform the tasks required.
* Sufficient computer skills required to perform duties.
* Registered Nursing degree & BSN preferred.
* Demonstrated proficiency in RAI/PPS processes.
