Job Description Responsibilities for the Case Manager include:Providing referrals to appropriate community resources Coordinating services to avoid duplication Monitoring client's progress toward goal achievement and periodically reassessing changes in health status Monitoring the care plan to ensure the effectiveness and appropriateness of services Acting as patient advocate by identifying and developing new community resources Maintaining accurate patient records Performing related duties and responsibilities as required
Required Skills 2+ years of geriatrics and home care experience Knowledge of managed care Solid computer skills
Desired Skills Spanish, Cantonese and/or Russian language skills
