Case Manager - Gibson and Dubois counties.

The primary focus of an IPMG Case Manager is advocacy with Hoosiers who have developmental disabilities and receive services through Medicaid waiver programs.
Responsible for coordination of supports, communicating with guardians, service providers and appropriate state agencies. Participate in implementation of Individual Support Plan through person centered planning. Monitor progress, outcomes, maintain records.
Case Managers work out of their homes and set flexible schedules. Initial training and support provided, travel required.
Bachelors degree required, prefer in a related field. Minimum 1 year experience working with people with developmental disabilities.
