The Case Manager has primary responsibility for reviewing each inpatient chart and identifying whether the patient meets and/or continues to meet admission criteria and working with the admitting physician to resolve issues. In addition, the Case Manager works closely with Medicare, Medicaid, and health plan utilization review personnel to ensure that the patient is qualified and that the hospital will be appropriately reimbursed for the inpatient stay. The Case Manager also has the primary responsibility for all discharge planning including working with the physician and the nursing staff to develop and execute a discharge plan that will provide for the patients clinical care needs after leaving the hospital.
