Locations include: New York City, Brooklyn, White Plains, New Jersey & work from home

Responsibilities:

*Provide telephonic case management of long-term health care services allowing plan members to remain in the community by collaborating with health care providers and developing personalized plans of care.

*Coordinate, authorize and track patient care for home care cases focusing on individuals who wish to remain at home.

*Provide case management including coordinating, authorization and tracking of patient care with accordance to the plan of care.

*Evaluate the effectiveness of the services within case management program and provide on-going follow-up with the patient, family, caregiver and physician.

Requirements:

RN, Registered Nurse

2+ years of experience in an acute care, home  care or managed care setting

Strong computer skills
