Cleveland - Local area government is currently seeking a degreed & licensed Case manager to support their Workforce Services division. Responsibilities of the position include: Provide intensive case management and employment-related services to hard-to-employ participants Develop Individual Service Plans Coordinates all project services provided to a participant Conduct assessments, identify barriers to employment, barrier resolution, participant contact, establishment of employment plans, co-case management with collaborative agencies, employment placement identification, and identification of support services and community resources Refer applicants and participants to appropriate program components and provide follow up for effectiveness Develop and use expertise to address special issues affecting participants, including, teen pregnancy, AODA, mental illness, homelessness, low literacy, and domestic violence Document, track and process participants' activities Prepare case summary for Professional Advisory Team review when necessary Meet established performance standards Provide quality customer service for internal and external customers Job Requirements: Bachelor's or Associate's Degree in health or human services field Two years experience in employment and training, or case management in an employment services program Must be licensed in his or her profession Proficiency in computer operations Excellent organizational, interpersonal, written, and verbal communication skills Ability to perform comfortably in a fast-paced, deadline-oriented work environment Ability to successfully execute many complex tasks simultaneously Ability to work as a team member, as well as independently Preferred qualifications include bilingual capabilities If you are interested in this position, and meet the listed requirements, please click on "apply now".
