Position Description: RN/LVN Care Manager coordinates the overall interdisciplinary plan of care for a patient, from admission to discharge. Responsible to collaborate with healthcare providers and members to promote quality member outcomes, to optimize member benefits, and to promote effective use of resources. Acts as a liaison between patient/family and healthcare personnel to ensure necessary care is provided promptly and effectively. Incumbent must be a RN or LVN licensed in the state of California with 2-4 years of hospital/acute care clinical experience. Familiar with standard concepts, practices, and procedures within a particular field. Relies on experience and judgment to plan and accomplish goals. Performs a variety of other tasks. A certain degree of creativity and latitude is required. Qualifications: Requires current and unrestricted Registered Nurse or LVN license in the State of California and at least 2 years of acute care clinical experience, as well as one year of recent case/care management and/or utilization review, and/or discharge planning and/or concurrent review experience. Strong PC/Computer knowledge, including MS Word, Excel, Outlook, IE, and Office is required. This position requires strong oral, written and interpersonal communication skills, problem solving, facilitation, and analytical skills.
