The Care Coordinator - MSHO is a home-officed position with extensive travel required in the Hinckley/Pine City/Mora/Onamia area. Responsible for assessing the medical and social needs of the MSHO product's members and ensuring they receive all necessary care and services. The assessment occurs either in their home or in nursing home settings. From the assessment, the Care Coordinator is responsible for creating appropriate treatment plans and coordinating the delivery of the medical and social services outlined in the care plan. Interfaces with physicians and other providers to discuss the treatment plan and provide updates on member progress as a result of implementing the treatment plan. In addition to the direct member interface, this position is part of an overall MSHO team within the Center for Healthy Aging and the incumbent will apply expertise to further the goals of the MSHO product. Also, this position is responsible for representing Medica at DHS care coordinator meetings and other related external meetings. Registered Nurse, Licensed Nurse with Bachelor's degree and/or equivalent health care experience, or Master level Social Worker with active licensure. 3+ years of clinical experience in public health setting including knowledge of community support resources. Experience in working with seniors (65+) - must work effectively with elderly population. Comfortable working with the low income population. PC skills including the Microsoft Office package. Ability to function independently and as a team player. Excellent communication skills with the ability to appropriately simplify medical language into a format understandable to non-medically trained members. Ability and desire to travel extensively throughout the local area.
