Position Description:
DESCRIPTION: The Care Manager is responsible for primary contact and liaison activities with both the qualified Medicaid recipients and community based providers in Nevada. This program engages in a collaborative approach with providers to ensure members are accessing and receiving services in an appropriate and timely manner. This program includes multiple systems of care including: State Mental Health, Child Welfare, Juvenile Justice, Substance Abuse and Educational Systems. • Providing telephonic care coordination of services and reducing any duplication of services by working with providers and case managers for identified programs • Activities include assessment, planning, implementing, coordinating, monitoring and evaluating the services and resources necessary to respond to an individual member’s behavioral health needs • Ensure that member’s are placed and maintained in the safest, lowest level of care Coordinate services to improve access to services and improve cost effectiveness of the delivery of services • Collecting clinical data and entering the information in APS CareConnection® for tracking and metrics reporting purposes • Communicating with providers in the community regarding the member’s treatment needs and developing appropriate care plans for specialized needs of members • Monitoring for quality of care and outcome improvement in accordance with clinical indicators as per contractual obligations • Assisting with Provider Education • Other duties as assigned. EDUCATION REQUIREMENTS: • Registered Nurse – Psych; Licensed Clinical Social Worker; with valid unrestricted license in Nevada. EXPERIENCE: • Minimum of 3 years experience in related public sector medical services including experience with children and adolescents in psychiatric/ residential treatment facilities, experience with quality improvement, medical record reviews, chronic/disease care management and/or primary care case management; • Experience managing members with chronic behavioral issues, goal setting, and providing constructive feedback preferred; • Strong verbal and written communication skills, prior experience in a training or educational capacity strongly desired; • Previous experience in delivering care coordination services to the Medicaid population preferred; • Above average computer/technical skills; • Exceptional communication skills with a customer service focus; and • Ability to resolve problems in a non-threatening manner.
