Home Health Services is a non-profit, community-based home care agency with six branch locations in the Dallas - Fort Worth, TX area and a branch office in Houston, TX.

Accredited by JCAHO and certified by Medicare, Home Health Services has been meeting the needs for home care specialized in serving the senior population since 1974.

We are currently seeking qualified candidates for an Area Manager for Onsite-Case Management at our office in the Houston area.

Reporting to the Regional Director Clinical Programs Houston, the Area Manager OCM will be a Business Development Specialist who will “Grow the Business” of the Agency. To that end, he or she will be focused on core deliverables such as increasing awareness of the agency in the field, bringing in new referrals, and coordinating public relations efforts. The focus is driving new business by strategic sourcing, development of referrals & retaining new client-base.

The OCM will also provide home health coordination, education and case management activities, while ensuring that referrals are coordinated safely.

Qualifications

A professional registered nurse or licensed social worker.Minimum of three (3) years experience as an RN or Licensed Social Worker with at least two years experience in the field of public health, community home care.Prior experience in discharge planning and supervision of employees preferred. Good working knowledge of PPS, Medicare, rules, regulations, marketing knowledge and experience in physician relationship building.Demonstration of consistent accountability for performance of professional nursing/social work care in the field of public health / community health.CCM certification helpful.Is self directed, able to work with little supervision making sound judgments / decisions, results oriented, and proactive in problem resolution.Is flexible and cooperative in fulfilling role obligations, traveling as business demands change.

 Responsibilities & Deliverables

1.         Spearhead growth and expansion of marketing and achieve set goals towards same. Provide and coordinate marketing presentations and other means of advertising to prospective clients.

 

2.         Create and execute market plan as developed with leadership team by maintaining high profile with accounts and prospects and timely follow-up on leads.

 

3.         Support and follow up on corporate PR and promotional initiatives.

 

 

Duties

 

Performs the following in the hospital, Skilled Nursing Facility or agency after the physician has determined that the patient requires home health services by the agency.

 

 

1.         Manages and facilitates the transfer of patients from a hospital or SNF to the care of the agency.  Confers with discharge planners and other involved professionals in the establishment of an individual patient management plan.

 

2.         Interviews individual clients and support systems/care providers to determine health needs and advocates for client needs as appropriate.

 

3.         Assists in establishing a definitive home care plan prior to hospital of SNF discharge.

           

4.         Arranges for the uninterrupted continuing medical care of the patient and assists in organizing family resources for the effective care of the patient.

 

5.         Monitors hospitalized patient status and pending discharge home care needs to 

            ensure continuity of care, reviews plan for consensus and plan modification if

            needed.  Notifies clinical staff as indicated.

 

6.         Confers with discharge planners, or others, when agency patients are readmitted to institutions to maintain and review continuity of care.

 

7.         Takes full and timely advantage of opportunities to market Home Health

            Services.

 

8.         Qualifies targets for HHS program presentations.

 

9.         Monitors accounts tenaciously for client satisfaction and makes improvement

            plans for overall agency performance improvement.
