This rewarding position offers the opportunity to work more closely with a smaller caseload, and it features the benefits and growth potential of a Fortune 50 healthcare organization. The hired Care Manager will determine the medical needs of high-acuity Medicare and Medicaid members.

 

Primary responsibilities include:Working with low-risk patients who have chronic conditions Performing health-risk assessments Setting up DMEs and making sure authorizations are approved Documenting all cases in our system Coordinating member care  Candidates must have a current, unrestricted LVN license in the state of Hawaii or have a degree in social work. General computer skills are necessary. The ability to have a strong phone presence and excellent communication skills is critical. A solid medical background is essential.
