The Clinical Case Manager will coordinate care for the elderly population and focus on decreasing hospitalizations. Additional responsibilities include:Managing home care for elderly enrollees on site and telephonically Updating documentation between visits as needed Scheduling appointments and visits with patients and ensuring proper care is given Attending weekly team meetings Focusing on disease management and decreasing hospitalizations for members
Requirements for this role are as follows:An RN with a current and unrestricted license in your state of residence Substantial clinical experience A background in case management in a home-care, community-health or an on-site setting Computer proficiency Excellent communication skills
